Care By Telehealth
Best suited for medication refills (non-opioids), follow up discussion on an existing condition you were treated for at our clinic (e.g., labs, CT or MRI results), cough, colds, pink eye, Covid-19, and Urinary Tract Infections.

- Submit your request here. Sorry, we don’t accept requests by phone.
- Expect to get a copy within 24 hours.
Medical Records
The law requires a written signature. As such, we must have a signed form to release the records. We only accepts requests from providers by phone. If you are a patient, attorney or a group issuing a subpoena, get started with your request online.
