Last walk in is 1 hour before closing and doors lock 15 minutes before posted closing time.

Health Exams

Sports Physicals, Pre-Operative Exam, TB Skin Test

• Mon – Fri: 10 am – 4:30 pm

Care By Telehealth

Best suited for medication refills (non-opioids), follow up discussion on an existing condition you were treated for at our clinic (e.g., labs, CT or MRI results), cough, colds, pink eye, Covid-19, and Urinary Tract Infections.

Lab

• Mon – Fri: 9 am – 4 pm

Discharge Notes

• Submit your request here. Sorry, we don’t accept requests by phone.
• Expect to get a copy within 24 hours.

Medical Records

The law requires a written signature. As such, we must have a signed form to release the records. We only accepts requests from providers by phone. If you are a patient, attorney or a group issuing a subpoena, get started with your request online.
